Well, it all depends on which aspects of the Cardiacs you'd like to have covered.
Fast complex stuff played with some kind of punk attitude can be found from, among others, Mr. Bungle, Dr. Nerve and Sleepytime Gorilla Museum.

Have a look at Tim Smith's influences, Gentle Giant, Slapp Happy/Henry Cow...'Nurses Whispering Verses' - a line lifted from 'In the Sickbay' (although not at all like Cardiacs...except maybe lyrically...)

Maybe the early Oingo Boingo (on a more straight-forward new wave direction)?
Some more evident choices:
- the Czech band Uz Jsme Doma (a bit darker and moodier than the Cardiacs, though);
- some projects of Yoshida Tatsuya: SaxRuins (with saxophonist Ryoko Ono);
- there were some post-punk bands that were somewhat close to the style of the Cardiacs, like Punishment of Luxury.

No one sounds like the Cardiacs! I've been seeking myself and no luck yet. That's a good thing though. Makes them utterly unique. Be sure to simply check out more Cardiacs including the EPs and live albums. THey are all worth hearing

I don't really know anyone like Cardiacs, I content myself with the associated bands, Spratley's Japs, Mr & Mrs Smith & Mr Drake, Tim Smith solo, Sea Nymphs, William D Drake solo. All are heavily influenced with the key member being Tim Smith in all cases except William D Drake solo.

Here's a good list of bands that are related to Cardiacs by membership if not sound


I can personally recommend, North Sea Radio Orchestra, Stars In Battledress & Guapo though none are anything like Cardiacs.

This link somehow doesn't mention Kavus Torabi's Knifeworld, which have some obvious connections to Cardiacs music (although not sure whether it's enough to classify as "sounding similar").

Knifeworld is great, but yes not really a sound alike other than wonky pop.
