That cover is just disgusting and immature - a cheap shocking trick - although offensive to some people. That...anti-motif was already seen in several horror movies. To the topic: Scarsick's cover is horrible, hands down. When I saw it for the first time on PA, I scrolled the page immediately Off the topic again: National Health's debut cover is spooky as well.
This is my all-time most horrifying cover: Laibach - Sympathy For The DevilNo skulls or decomposing corpses, no fetuses or Satan. Just a picture of Shiny Happy Family - but in Nazi-propaganda style. It's not the content of the picture, it's the implication and threatening that scares me. Maybe it's just me...dunno, It sends shivers down my spine every time I look at it.

I'll go for the Scarsick cover, mainly because its not that good a cover.

None of these two are particularily creepy in my opinion. The PT cover
is nice, but nothing really special, and the PoS cover is just plain
bad.

The worst is the Scorpions' Virgin Killer...I mean, come on guys, even in the early 70s that was pretty bad. Good band, though.
